Great but constantly disconnects

Its great when its working but the app constantly disconnects from my sensor. The receiver is far less prone to disconnecting.

Could you also add the ability to add a text note (much like I can annotate when I put on insulin). This can help me keep track of insulin site locations and their effectiveness. Notes for other activities that = sugar level. Food factors, sleep... Etc.
